Support at Home Services

To ensure that support is targeted to individual needs, Support at Home offers a clear list of services under three distinct service categories, each funded slightly differently depending on your financial situation.

This category includes nursing care such as wound care, medication management and continence care. It also includes allied health services like physiotherapy, occupational therapy and podiatry.

Regardless of your financial situation, the government will fully cover the cost of all clinical supports, so there is no out-of-pocket cost.

This category includes help with daily activities such as personal care (showering and dressing), social support and community engagement (transport to appointments), and respite care for carers to take a break.

Depending on your financial situation, you will partially cover the cost of Independence Support through a co-contribution.

Please Note: If you are a Grandfathered HCP recipient (approved for a Home Care Package before 12 September 2024) you will be no worse off under the contribution arrangements for Support at Home. You will never pay more than you are currently paying with an ITF. 

This category covers essential services to support your independence to maintain a clean and liveable home environment. For example, assistance with cleaning, gardening, shopping, or meal preparation. 

Depending on your financial situation, you will partially cover the cost of Everyday Living Support through a co-contribution.

CCA Fee Example

CCA has moved from a flat 11% coordination fee to a service utilisation rate. Here’s how it works in practice:

Support at Home – Everything You Need to Know 

Is Support at Home the same as the old Home Care Package program?

Support at Home will replace the current Home Care Package (HCP) system from 1 November 2025. 

If you already receive a Home Care Package, you will be automatically transitioned to an equivalent Support at Home level. There is no interruption to your services, and your funding simply shifts to the new system.

Support at Home introduces a simpler, more flexible, and more personalised approach to aged care. Key improvements include:

  • Eight levels of ongoing support, plus multiple short-term pathways
    This ensures your funding aligns closely with your individual health, safety, and daily living needs.

  • Upfront funding for Home Modifications and Assistive Technology
    Approved equipment and home upgrades can now be funded upfront, meaning you no longer need to save money from your care budget to access essential support items.

  • Capped Care Management costs
    A set portion of your funding is allocated to your Care Manager, ensuring you receive the guidance and support needed to manage your services.

  • Fair and transparent participant contributions
    You’ll only contribute to the services you actually use. Contributions are based on a set percentage of each service’s hourly cost.

For example: if you receive two hours of personal care, your contribution applies only to those two hours.

Yes, some services require a co-contribution. The amount depends on:

  • The type of service (clinical services will be fully covered) 
  • Your financial situation 
  • The number of hours or units of service you receive

You only pay for what you use. 
If you received a Home Care Package before 12 September 2024, your contribution arrangements will not be made worse under Support at Home. 

Support at Home groups services into three main categories:

  1. Clinical Care
    Nursing, wound care, medication management, continence support, allied health (e.g., exercise physiology, physiotherapy, podiatry), and other health-related services.
  2. Independence Support
    Personal care, assistance with mobility, nutrition and meal preparation, medication prompting, and support to maintain daily living activities.
  3. Everyday Living Support
    Domestic assistance, laundry, transport, shopping, social support, community participation, and companionship.


You may also receive additional funding for Assistive Technology and Home Modifications when needed to improve safety and independence.

If your health or circumstances change, you can request a reassessment at any time.
Your support level may be adjusted to better meet your needs, and your care plan will be updated accordingly.

If you are currently on the waiting list, your place is protected. When Support at Home begins, your approved HCP level will convert directly to an equivalent Support at Home level.

You will not lose your position or progress in the queue.

  • Start with My Aged Care — Call or apply online for an assessment.

  • Complete your assessment — A trained assessor will visit you to determine eligibility and care needs.

  • Receive your support level — You’ll be assigned a Support at Home level and quarterly budget.

  • Choose your provider — Call CCA, and we’ll help you find a provider, design a personalised care plan and coordinate your services.

Yes. Support at Home is designed to increase flexibility and choice.
You can choose who delivers your care, and your Care Coordinator can help organise trusted, local providers who best match your needs and preferences.

Under Support at Home, funding is provided quarterly rather than annually. This ensures a more predictable and stable budgeting cycle. Unused funds do not carry over as they currently do in HCP — instead, the system focuses on matching funding to your ongoing needs.

Support at Home Additional Funding

Restorative Care Short Term Pathway

Up to $6,000

Up to $6,000 (16 weeks) for extra nursing and allied health support to help you regain strength and independence after illness, injury, or a hospital stay.

End-of-Life Short Term Pathway

Up to $25,000

Up to $25,000 (12–16 weeks) for additional in-home care for people nearing the end of life to support with palliative care at home. 

Assistive Technology and Home Modification

Up to $15,000

Up to $15,000 in funds to spend on assistive technology and/or home modifications (AT–HM) to help participants live more independently and safely by providing tools like mobility aids or physical changes to their homes, such as grab rails or ramps.
