Our coordination team ensures every dollar of your funding goes further, connecting you with trusted local services and tailored support that enhances your quality of life.
At CCA, we believe that when your funding is prioritised, you stay independent, supported, and in control of your life for longer.
As independent coordinators, our role is simple: to help you make the most of your Support at Home funding, connect you with trusted local providers, and ensure every dollar goes toward improving your wellbeing.
Under Support at Home’s clinical care stream, we work closely with Exercise Physiologists, Physiotherapists, Occupational Therapists, Dietitians, and other allied health professionals to:
With CCA, your care isn’t managed by a call centre or hidden behind high provider fees, it’s personally coordinated by a team that represents you.
After facing the hurdles of My Aged Care with their own family, Kaelan set out to make care more transparent, accessible, and empowering for others alongside his Mum, Tania.
Our mission is to make home care transparent, empowering, and enjoyable, so you can continue living safely and independently where you belong, at home.
To ensure that support is targeted to individual needs, Support at Home offers a clear list of services under three distinct service categories, each funded slightly differently depending on your financial situation.
Click each section to see inclusions.
This category includes nursing care such as wound care, medication management and continence care. It also includes allied health services like physiotherapy, occupational therapy and podiatry.
Regardless of your financial situation, the government will fully cover the cost of all clinical supports, so there is no out-of-pocket cost.
This category includes help with daily activities such as personal care (showering and dressing), social support and community engagement (transport to appointments), and respite care for carers to take a break.
Depending on your financial situation, you will partially cover the cost of Independence Support through a co-contribution.
Please Note: If you are a Grandfathered HCP recipient (approved for a Home Care Package before 12 September 2024) you will be no worse off under the contribution arrangements for Support at Home. You will never pay more than you are currently paying with an ITF.
This category covers essential services to support your independence to maintain a clean and liveable home environment. For example, assistance with cleaning, gardening, shopping, or meal preparation.
Depending on your financial situation, you will partially cover the cost of Everyday Living Support through a co-contribution.
Most people don’t realise how easy it is to change providers, until they do it. What do they notice straight away? The difference.
Care recipient’s tell us it feels like a weight has lifted.
They spend less time navigating care and more time actually enjoying it.
We make the process easy, so you can focus on what matters most.
We’ll talk through your goals, answer any questions, and complete the initial onboarding form together.
Once you’re onboard, we start lining up the care and support that fits your life, and your budget.
Understand what the government funds, when co-contributions apply, and how CCA helps you stretch every dollar.
Under Support at Home, CCA charges in line with each service received instead of a set monthly percentage. This utilisation-based structure means you only pay coordination when you receive care.
| Service | CCA Advantage | Fully Managed Providers |
|---|---|---|
| Management Costs | 10% Care Management & 10% Provider Utilisation Rate | Minimum 10% Care Management & 10% Provider Utilisation Rate |
| Care Management | 25% utilisation rate | From $200.50/hr |
| Nursing | From $100/hr | From $224/hr |
| Allied Health | From $115/hr | From $268/hr |
| Personal Care | From $65/hr | From $135/hr |
| Social Support | From $50/hr | From $135/hr |
| Transport | From $10 per trip | From $85 per trip |
| Meal Preparation | Freedom of choice | From $135/hr |
| Overall | Affordable, personalised, transparent | Higher fees, less flexibility, provider-controlled services |
CCA has moved from a flat 11% coordination fee to a service utilisation rate. Here’s how it works in practice:
| Item | Amount |
|---|---|
| Supplier service price | $50.00 |
| + Care Coordination Service Utilisation Rate (25%) | $12.50 |
| + Provider Fee (10%) | $6.25 |
| = Total Hourly Cost | $68.75 |
Note: This is an example for a standard hourly service. Actual pricing varies by service provider rates and location.
Under Support at Home, your funding will no longer accrue into an “unspent funds” account. For equipment (Assistive Technology) and home modifications, we will apply for these together through My Aged Care, with coordination fees capped at:
We’re proud to have helped so many families find the right care solutions.
But don’t just take our word for it – here’s what some of our clients have to say about their experience with Care Coordination Australia.
After relocating to a new area in the Wide Bay region, CCA helped establish services in our home that allowed us to deal with my spinal injury. All the services arranged were completed quickly to suit my disability. CCA has made me feel independent and are a team I can trust. CCA has allowed us to feel safe and enjoy our lives to the fullest.
We recommend that any Support at Home recipients that need help and guidance should contact CCA. You will notice the difference with their professional approach, which will be very beneficial to your life’s progress.

My wife and I appointed CCA as our coordinator’s , 18 months ago. I am happy to say I am totally happy with their service. In May 2024 my wife was diagnosed with AML Leukemia and was weeks to live. I immediately advised CCA asking for additional help. The team were so helpful and compassionate, and immediately helped where possible.
My wife and I were so surprised of their response. After my wife’s passing CCA have been so caring, keeping in contact with myself, offering additional support if required. Being with CCA is like belonging to one big happy family. Caring and compassionate. I have recommended them to a number of my friends, and they are all happy.

The care coordination team is exceptional in their client-focused approach. They ensure that all resources, including support personnel and necessary supplies, are allocated and procured well ahead of time, providing a seamless and stress-free experience for clients.
Their proactive approach to resolving issues and their unwavering commitment to advocating for clients is truly commendable. They go above and beyond to ensure that every detail is managed with care and precision. I would highly recommend their services to anyone looking for compassionate and professional support.

Clinical Nurse
We first started being coordinated by CCA in 2022. We are pleased to return to CCA after leaving to go to a local provider. We were promised a lot but got left without carers for five days. They were hard to contact, and we knew we needed to change back to CCA.
Care was reinstated immediately, and we have a wonderful care coordinator and very caring local carers. We could not be happier.

HCP Recipients
Working independently with CCA has allowed myself the freedom to develop a strong relationship and friendship with my client. When I first met Ron, he was under a lot of stress with his wife in care. Now Ron visits regularly, has social outings and domestic assistance which has greatly improved his quality of life and mental health. This has been noticed by his family and friends.
I truly believe that many age care recipients would greatly benefit from this type of support in the future of age care, compared to the more traditional and less personable support that has been provided by larger institutions.

Support at Home – Everything You Need to Know
Support at Home will replace the current Home Care Package (HCP) system from 1 November 2025.
If you already receive a Home Care Package, you will be automatically transitioned to an equivalent Support at Home level. There is no interruption to your services, and your funding simply shifts to the new system.
Support at Home introduces a simpler, more flexible, and more personalised approach to aged care. Key improvements include:
For example: if you receive two hours of personal care, your contribution applies only to those two hours.
Yes, some services require a co-contribution. The amount depends on:
You only pay for what you use.
If you received a Home Care Package before 12 September 2024, your contribution arrangements will not be made worse under Support at Home.
Support at Home groups services into three main categories:
You may also receive additional funding for Assistive Technology and Home Modifications when needed to improve safety and independence.
If your health or circumstances change, you can request a reassessment at any time.
Your support level may be adjusted to better meet your needs, and your care plan will be updated accordingly.
If you are currently on the waiting list, your place is protected. When Support at Home begins, your approved HCP level will convert directly to an equivalent Support at Home level.
You will not lose your position or progress in the queue.
Yes. Support at Home is designed to increase flexibility and choice.
You can choose who delivers your care, and your Care Coordinator can help organise trusted, local providers who best match your needs and preferences.
Under Support at Home, funding is provided quarterly rather than annually. This ensures a more predictable and stable budgeting cycle. Unused funds do not carry over as they currently do in HCP — instead, the system focuses on matching funding to your ongoing needs.
CCA helps you:
